AFTERMATH
Evidence-first failure prevention

Every failure should leave a safeguard.

Turn incident evidence into actionable safeguards. Understand what happened, identify what remains uncertain, and preserve lessons that can be retrieved when similar failures occur.

Incident material

A log line, an error, a failed request — treated strictly as data.

Derived safeguard

One specific, testable rule — with its assumptions and its verification state stated, never assumed.

Submit an incident, get a Safeguard Record.

The material is treated strictly as data: parsed with rules, never executed, and never interpolated into a command or a query.

required · 0/20,000

Paste the real material — an error, a stack trace, a log excerpt, a failed request. It is parsed with rules and never executed. Do not paste secrets.

You will get a Safeguard Record: evidence, hypotheses, a proposed safeguard, and how to verify it.
How it works

One chain, six links, no shortcuts.

A record is a single connected argument. Each link carries an honest state label, and a claim is never upgraded as it moves down the chain.

  1. Incident

    the submitted material

Treated strictly as data. Parsed with rules, never executed.

  2. Evidence

    verbatim spans

Observed facts separated from what a source confirmed.

  3. Hypothesis

    the observed facts

A candidate mechanism with a confidence and a list of unknowns.

  4. Safeguard

    the diagnosis

One specific, testable rule — with its assumptions stated.

  5. Verification

    the safeguard

Executed against the decision engine. Never asserted.

  6. Memory

    the verified record

Preserved with a deterministic key, retrievable later.

Principles

What makes AFTERMATH different.

01

Evidence before conclusions

Every fact in a record is extracted from the submitted material with a verbatim span behind it. The engine forms a diagnosis only after it has stated what it observed.

02

Uncertainty is explicit

What is observed, what an external source confirmed, and what is still unknown are three separate lists. A lookup that cannot run resolves to unknown — never to success.

03

A safeguard is not called verified without proof

A safeguard is recommended, then tested against the decision engine. A single incident can show an effect; it can never on its own justify calling a rule verified.

04

Lessons can be preserved and retrieved

Each record is written to Walrus Memory with a deterministic key, so a later, similar failure can recall the lesson — and a repeat is recognised as a repeat.

Live system state

Infrastructure metadata and the controlled evaluation scenarios live in the Evaluation Lab, away from the primary journey. Every figure there is read from the running system; nothing is illustrative.
